Arrival-Departure Record (I-94)
If you have left the U.S. and still have your I-94 or I-94W, it means that your departure was not recorded properly. In order to avoid any problems in the future, you should forward the form to the appropriate authorities so that your record is corrected.
Information on how to record your departure may be obtained through the Department of Homeland Security Bureau of Customs and Border Protection (BCBP) website at the following link: http://cbp.gov/xp/cgov/travel/id_visa/.
Attention Visa Waiver Program (VWP) Travelers:
U.S. Customs and Border Protection has recently begun a transition to paperless processing for VWP travelers who have obtained a travel authorization via the Electronic System for Travel Authorization (ESTA). However, it is important to note the I-94W form will continue to be required at land borders, in the case of system outages, and at ports of entry that have not completed the transition to automated processing.
